My Choice Family Care, a managed care organization (MCO) serving approximately 8,400 elderly and physically, intellectually, and developmentally disabled persons, has been selected to move forward for Family Care certification in Rock County, effective July 1, 2016.
Family Care is a program geared toward helping individuals maximize their independence and sense of community connectedness while increasing their personal ownership health.
My Choice Family Care is currently contracted with the State of Wisconsin Department of Health Services (DHS) to offer the Family Care program in Milwaukee, Racine, Kenosha, Washington, Ozaukee, Sheboygan, Waukesha, and Walworth counties. The MCO is now in the process of expanding its provider network to serve people in Rock County. Following certification by DHS, My Choice Family Care will be one of two managed care options for people to receive their community-based long term care benefit.
